Nomination Requirements
Thank you for nominating a loved one for a service, treatment or product! Please read the following requirements prior to submitting a nomination. Any submissions that do not adhere to our requirements will not be considered.
NOMINEE REQUIREMENTS
We accept nominations for the following:
Cancer patients currently undergoing oncology or chemotherapy treatments
Cancer survivors who are no longer undergoing any cancer treatments
SERVICE REQUIREMENTS
Our services offered depend on what stage of treatment/recovery the nominee is currently in. Please ensure your nominee is in the correct stage prior to completing the form:
Oncology Facial: for those currently undergoing chemotherapy or radiation treatments. **Available for nominees in Denver, CO only.
Scar Revision/Reduction: **Available for nominees in Denver, CO only.
Permanent Makeup - Brows: **Available for nominees in Denver, CO only.
3D Areola Restoration Tattooing: **Available for nominees in most major cities.
Scar Reduction Products: **Available for nominees nationwide (we’ll ship the product to them!)
iS Clinical PURE WELLNESS Skincare Kit: for those currently undergoing or have recently completed chemotherapy or radiation treatments within the last year. **Available for nominees nationwide (we’ll ship the product to them!)
